Disability and FMLA form completion is handled by HealthMark Group, our medical records partner. There is a fee for this service, determined and collected by HealthMark directly.

How It Works

Form Completion by HealthMark Group

Neuroscience Specialists uses HealthMark Group to complete all FMLA and disability form requests. Our office does not complete these forms directly. Once you submit your request to HealthMark, they coordinate with your physician, your employer, and your insurance company on your behalf. The entire process takes approximately seven business days.

Step-by-Step Timeline

On average, FMLA or disability form completion takes approximately seven days from the time you submit your request.

Submit your request
Email your completed form to fmla@healthmark-group.com with your full name, date of birth, physician name, and facility name and address. You can also submit through HealthMark's portal.

Receive an invoice (Day 1)
HealthMark will verify your information and send you an invoice. The fee is determined by HealthMark and must be paid before your form is processed. Most insurance providers do not cover this cost, but you may be able to seek reimbursement from your employer.

Form completion (Days 1–3)
After payment, HealthMark completes your form using clinical experts and coordinates with your physician's office for review and signature.

Physician signature (Days 3–5)
The completed form is sent to your physician for signature, then returned to HealthMark. Please do not contact our office to check the status — all updates are handled through HealthMark's Request Manager.

Form released (Day 7)
You will receive a notification when your completed form is available for download at requestmanager.healthmark-group.com. We recommend downloading a copy for your own records.

Frequently Asked Questions

Why is there a fee for form completion?

Completing FMLA and disability forms requires clinical expertise, coordination with your insurance company, your employer, and your physician, and strict HIPAA-compliant handling. The fee charged by HealthMark covers those resources. Most insurance plans do not cover this cost, but you may be able to submit a receipt for reimbursement from your employer.

I received an invoice but didn't request a form.

Your employer or insurance company may have contacted your physician directly to initiate the form without notifying you first. Even if you did not start the request, you are still responsible for working with HealthMark to complete the process.

Something on my completed form needs to be corrected.

Email fmla@healthmark-group.com with the details of what needs to be updated. Do not contact our office for corrections — HealthMark manages all updates and our office cannot process them directly.

Can I have more than one form completed?

Yes. Include all forms in the same email submission. Note that additional forms or forms exceeding five pages may incur additional charges.

My insurance company says they didn't receive the form.

HealthMark can re-send the completed form, and you can forward it directly to any party who needs a copy. We strongly recommend having your completed forms released directly to you to avoid delays caused by large insurance company mail queues.
