Referral Process

How to Refer a Patient

Call or fax the referral
Contact our office by phone at (405) 748-3300 or fax your referral to our workers' compensation intake line. Please indicate it is a workers' comp case.

Include key information
Send the claim number, carrier information, authorization number (if required), prior imaging, prior treatment records, and the mechanism of injury.

Scheduling confirmation
Our workers' comp scheduling coordinator will contact the patient directly and confirm the appointment with your office.

Pre-appointment
We obtain and review all prior imaging and records before the appointment so the consultation is productive and no time is wasted.

Post-appointment reporting
Reports are generated promptly after the evaluation. We will contact you with any urgent findings. Standard reporting turnaround is within 5–7 business days.
