The fastest way to access your medical records is through our secure Patient Portal. Visit summaries, lab results, imaging reports, and other health information are available to you 24/7 at no charge.

How to Request Records

Records via HealthMark Group

If you need records sent to a third party — such as another provider, attorney, or insurer — Neuroscience Specialists partners with HealthMark Group to fulfill those requests. This service is provided at no charge to the patient. Records are sent via email by default; a hard copy is available upon request.

Visit the HealthMark Group portal
Navigate to the HealthMark Group request portal to begin your records request online.

Submit your request
Select "Submit Request" from the menu and complete all required fields to authorize HealthMark to retrieve your records.

Receive notification
You will be notified by mail or email when your records are complete and ready for download.

Log in to the HealthMark Request Manager for status updates or to chat with support.

Additional Notes

Records Information

  • Records provided to other treating physicians for continuity of care are typically sent directly at no charge — contact our office
  • Disability & FMLA forms are processed through HealthMark Group (a fee applies) — our office does not complete these forms directly
  • Imaging discs (MRI, CT, X-ray) may be requested directly from the imaging facility where the study was performed
  • Authorized representatives or legal guardians may request records on a patient's behalf with appropriate documentation
